1. Medical Consent

I authorize the Divine Connections Retreat organizers and designated staff to provide, seek, and consent to necessary medical treatment on my behalf in the event of an emergency. I understand that I am responsible for any medical expenses incurred.

2. Acknowledgment of Risks

I acknowledge that participation in the Divine Connections Retreat may involve physical activities such as hiking, sports, and other outdoor events. I understand the nature of these activities and accept the risks involved.

3. Behavioral Expectations

I agree to follow the rules, guidelines, and instructions provided by the retreat organizers and staff at all times. I understand that respectful behavior is expected to ensure the safety and comfort of all participants.

Declaration

By accepting this Consent Form during registration, I have read and understood this consent form. I agree to its terms voluntarily and knowingly.

For participants under 18, a parent or guardian must accept this form on the participant's behalf during registration.
